Adoption Application

Thank you for your interest in adopting a dog from Gimme Shelter Animal Rescue. Our goal is to find the perfect forever home for each dog based on their individual needs. We believe that every dog deserves a safe and loving home, companionship, proper medical care, nutrition, exercise and training. We thoroughly screen each potential adopter for the safety of our animals and to ensure the best possible home for each dog. The screening process includes completing this application, followed by a phone interview and a home visit. We do not place dogs in the order the application is received. We choose homes that best fit the needs of each dog. We do our best to move through the process quickly but depending on the volume of applications it may take 5 to 7 days before we contact you. We try to contact every applicant. But in the event you do not hear from us in 10 to 14 days you should assume that you were not a match for the dog you applied for.

Before completing your application please note:

* We do not adopt to homes with unaltered pets.

* if you live in a house we require the yard to be fully fenced. (Not electric fence)

* All references listed will be contacted so please notify them that we will be in touch.

* We call your vet reference to confirm your current pets are spayed/neutered, up-to-date on mandatory vaccines, heartworm testing and preventative. If you do not bring your pets to the vet for vaccines, please provide name and phone number of where you take them for those shots. If you do not have an animal please provide the name and number of the vet who treated your previous animal.

*You must be within a reasonable driving distance of the dog you are interested in adopting. We do not transport dogs to you. You will be expected to make the trip to the foster home to meet the dog.

* We do not waive or adjust the adoption fee. If you cannot afford the fee it is doubtful that you will be able to afford medical care and/or emergencies that will arise during the normal course of a dogs life.

* Completing an application does not guarantee that you will receive the animal you applied for. We reserve the right to reject any application as we see fit.

If you do not agree with or meet our requirements please do not submit an application.

Make sure to fill out every field or application will not submit. A confirmation page and follow up email means that your submission was successful.

Under 6 months: $425 | 6 mos – 5 years: $375 | Over 5 years: $275



Medical records are provided upon adoption documenting all the vetting the animal has received.

I have read the above information carefully and filled out this application honestly. I understand that omission of information and/or failure to tell the truth or answer all questions fully and honestly will result in this application not being processed and/or being denied. Also, if an omission or untruth is discovered after an adoption takes place, I understand that GIMME SHELTER Animal Rescue (GSAR) reserves the right to annul the adoption and reclaim the animal. I give GSAR permission to fully investigate the information provided as well as to contact veterinarians, landlords, references, and other relevant parties disclosed on this application. If this application is submitted to GSAR from the Applicant’s email address then the application will be deemed to be signed.

If this application passes review, GSAR reserves the right to also conduct a home visit as part of the approval process and before any adoption is confirmed. I understand the approval is dependent on many factors, including but not limited to, the compatibility of the family and home to the individual animal, as well as other applications received on this animal. I understand it is GSAR’s sole prerogative to decide which home is most appropriate and that their decision is final; therefore I will not argue with the decision.


You will be required to provide a copy of this license upon signing the adoption contract.
